Hardship Fund Forms

The Hardship Fund is a resource for faculty and staff who are experiencing a financial crisis. You will need to speak with an EAP coordinator either in person or by phone prior to filling out these forms to get a better sense if you may qualify, and to get assistance with the application process if needed. The EAP coordinator may refer you to additional resources that may be able to help with resolving your situation.

To apply fill out appropriate forms to the best of your ability. You will be required to provide supporting documentation that demonstrates the hardship.

• W9 Form
  • The W9 form requires the information of the payee. This should be the name and address of the company/bill that you want to be paid. Do not fill in your own information on the W9 form (unless approved by EAP Coordinator).
