Online Forms

Student intake form

Students requesting accommodations should start by completing the Online Student Intake Form. It is strongly suggested that students submit this form along with their documentation in order to expedite the registration process.

The Online Student Intake Form requires an SBU NetID login. Students who do not have a NetID should complete the PDF or DOC Student Intake Form which they can download below.

Documentation

In addition to the Student Intake Form, students should read the descriptions of the documentation forms available below and submit the appropriate form(s). Students will need to work with a treating professional in order to complete the documentation forms.  Any questions about documentation should be directed to the SASC at SASC@stonybrook.edu or at 631-632-6748.

All forms should be submitted to the SASC by email (SASC@stonybrook.edu) or by fax (631-632-6747). In order to process forms, students should be sure to include their SBU ID Number and requested contact information on each form

Classroom, Testing, Accessibility, and Mobility Accommodations

Students requesting classroom, testing, accessibility, mobility, or any other non-housing related accommodation should work with a treatment professional to complete the Documentation of Disability Form.  Care should be taken to ensure the treatment provider includes all information requested on the form.

Residence Hall Accommodations

 Residential students that have secured campus housing that are requesting housing accommodations should work with their treatment provider to complete the Documentation for Housing Accommodation Requests Form. This form should also be used to request any accommodations related to campus dining services. 

If the request for accommodation is made after June 21st, SBU cannot guarantee that it will be able to meet the individual’s accommodation needs during the first semester or term of occupancy. Please note that submitting a request for accommodation is not a guarantee you will receive the requested accommodations

Accessible Parking Pass

Students who need an Accessible Parking Pass should work with their treating professional to complete the Temporary Accessible Parking Request Form. Students should ensure that they provide the complete information requested, including the information about their vehicle. SASC can issue campus-only accessible parking passes for up to 4 weeks.

Emotional Support Animals 

Students requesting an emotional support animal should work with a treatment professional to complete the ESA Request Form.  Care should be taken to ensure the treatment provider includes all information requested on the form.

aDDITIONAL fORMS

Release Form: In evaluating a student's documentation or disability, the SASC may require additional information from a treating professional.  In order to request this information, the student must ask the treating professional to release that information.  This form should be completed in order to authorize that information to be released to the Student Accessibility Support Center.

Paratransit Schedule: Students may request mobility assistance through SBU's para-transportation bus service by completing the Para-Transportation Schedule and submitting it with accompanying Student Intake Information and Documentation of Disability Forms. Students who have already provided documentation in support of Para-Transportation Services can update their transportation schedule by submitting a new form.

Alternative Text Form: 
Students seeking alternative access to print materials and students requesting real-time captioning or sign language interpreting should notify the Student Accessibility Support Center with sufficient time to ensure access.
